Behavioral Health Home Program Manager

Program Manager – Professional with Master’s Degree to provide leadership to the Behavioral Health Home Team.  Seeking administrator with behavioral health knowledge required and primary care knowledge preferred.  Responsible for the overall implementation and coordination of the Behavioral Health Home within United Services. Develop and maintain working relationships with primary and specialty care providers including both outpatient and inpatient facilities.  Development of health promotion and wellness initiatives.  Monitor and report performance, outcomes, and lead improvement efforts. Utilize population health data to inform practice.  Provide training and supervision to BHH team members including nurse care managers.

Provide supervision to staff in the Behavioral Health Home model of services that has been implemented in CT as a part of the Affordable Care Act.  The Behavioral Health Home (BHH) is intended to integrate behavioral health care with medical care for adults with serious mental health issues who also have serious and chronic health conditions.  Through an array of services the goal is to increase access to care, improve the experience of care, and to reduce unnecessary emergency and inpatient care while improving health outcomes.
