Peer Recovery Specialist – Behavioral Health Home

Full-time position available in the Dayville area.  Associates Degree or High School Diploma/Equivalent plus two years relevant experience required to assist participants in following up with medical care such as appointment reminders, accompaniment to medical appointments, obtaining prescriptions, ensuring basic needs are met including when being discharged from an inpatient stay.  Support and coach participants regarding treatment adherence, healthy eating and physical activity, restful sleep, ADL’s, stress management and developing a support network. Assist clients in linking and following through with self-help, recovery, and wellness groups and activities including transporting and accompanying clients to such activities. Assist in conducting wellness and health promotion activities.  Model successful recovery strategies for peers.  Must be certified as a Recovery Specialist through Advocacy Unlimited.

Schedule: Monday through Friday, including occasional evenings as needed based on program need.

Seeking applicant to staff a new model of services being implemented in CT as a part of the Affordable Care Act. The Behavioral Health Home (BHH) is intended to integrate behavioral health care with medical care for adults with serious mental health issues who also have serious and chronic health conditions.  Through an array of services the goal is to increase access to care, improve the experience of care, and to reduce unnecessary emergency and inpatient care while improving health outcomes.

Salary Range:

$12.60 – $14.26 per hour depending on experience.
