Description:
United Services is seeking a hospital liaison to provide a higher level of care coordination to the clients we serve. The qualified candidate will provide outreach, engagement, and coordination as assigned by the program manager to at-risk or difficult-to-engage clients and represent USI at external meetings to coordinate client care, including dispositions of client care. As a Liaison at United Services, Inc., you will play a vital role in connecting individuals with the resources they need for mental health and wellness. This position is essential for fostering relationships within the community and ensuring that our services are accessible to those who require support.
Schedule:
Monday – Friday 9:00 am – 5:30 pm or As needed
Minimum Requirements:
Bachelor’s Degree or Associate’s Degree in a relevant human services field with 2 years of relevant experience.
Salary Range:
- Hospital Liaison Salary: $18.54 – $20.98 per hour
- Bilingual English/Spanish fluency: $19.48 – $22.04
