Description:
United Services seeks a Family Nurse Practitioner (FNP) in our Primary Care Program. Our FNP will provide care to individuals and families of diverse backgrounds throughout the client’s lifespan, concentrating on the promotion of health, patient education, and comprehensive care. The Family Nurse Practitioner will focus on client-centered medical services, including assessments, evaluations, diagnosis, treatment planning, and medication administration. They will collaborate with other interdisciplinary team members while providing leadership and direction to non-medical staff in providing quality primary care.
Minimum Requirements:
- Current APRN/FPN license and certification required by the state of Connecticut.
- Master’s degree in nursing and a minimum of two years’ experience in a community health facility is strongly preferred.
- Membership in the American Nurse Association and a history of no adverse actions in the National Practitioner Data Bank.
- Ability to obtain and maintain medical credentialing and the ability to obtain and maintain federal and state narcotic registration to prescribe schedule drugs in class II, III, IV, and V.
- Excellent medical, clinical assessment, crisis intervention, medication administration, and service coordination skills required.
- Excellent interpersonal and communication skills, both oral and written, cultural sensitivity, and the ability to function as a member of an interdisciplinary team. Valid driver’s license and reliable vehicle is required.
- Fluency in English/Spanish preferred.
Schedule:
Monday – Friday 8:30am – 5:00pm
